How's the timing doing, have you put a light on it to see if you jumped a timing belt. Are your plugs ok? Usually a backfire in exhaust is raw fuel burning in the exhaust pipe.
It can be several things, maybe some more clues on how it acts, have you made any changes, how is the history of the truck and tune ups. Does it always do it consistently or only when warmed up. Have you done a visual of engine to see if anything has disconnected. Could be MCA jet valve leaking and letting unburned fuel down exhaust and then igniting it because a valve is partially open. Does it have power, compression tested it lately? Choke fully opened after warmed up.
Try to gather the evidence on it as much as possible.
